Your First Oral Surgery Visit

Prior to your appointment

We are proud to have been one of the first office’s to convert to paperless medical records in the 1990’s. We ask you to complete your health history and other registration forms online and securely submit them to the office before you arrive.

This will save you valuable time and help us prepare for your appointment. Please make every effort to arrive on time. Dr. Jacobs, Dr. Kirsch and Dr. Levin take great pride in being on-time for scheduled appointments.

At your first appointment

At your consultation you will meet privately with the doctor who will review your medical history and go over the reason for your visit. After the doctors examination, they will discuss your treatment options. This appointment will last however long you need to make sure you fully understand your condition and choices.

Certain oral surgery procedures are covered under your dental benefits. Our team of insurance professionals with help you maximize your insurance benefits. In order to do this, we need your dental insurance information as well as photo identification at your first visit. After your consultation appointment our benefits coordinator can contact your insurance to determine your benefits for the planned procedure.

  • All patients under the age of 18 will need to be accompanied by a parent or legal guardian.
  • A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. Please have nothing to eat or drink eight hours prior to your surgery. You will also need an adult to drive you home.
  • Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

For your convenience, the NJ Center for Oral Surgery accepts Checks, Visa, MasterCard, American Express, CareCredit and Lending Club. We deliver the finest care, in a comfortable, modern environment using the best materials at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. The NJ Center of Oral Surgery understands that temporary situations may arise which may affect timely payment on your account. If this happens, simply notify the office so they can help you. Many times, a simple telephone call can eliminate all difficulties.

If your dentist or physician has taken x-rays you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary they can be taken at our facility.

Yes, we accept FSA and HSA cards for payments. Please check with your benefits provider to ensure the expenses are eligible under your plan.

If your insurance doesn’t cover the full cost, we’ll work with you to review payment options. We offer flexible payment plans and financing options to help make your treatment affordable.

If you’re running late, please call our office as soon as possible. We’ll do our best to accommodate you, but depending on our schedule, we may need to reschedule your appointment to avoid delays for other patients.

Yes, another adult can accompany you, but for patients under 18, a parent or legal guardian must be present. For surgical procedures, we recommend having a responsible adult drive you home and stay with you as you recover.
